Screen Layout suggestion needed - any volunteers ?

I'm working on a small app, that requires user to input some data, in sort of like a form...and I'm looking for suggestions on screen-flows, and design ideas.

I love the TouchArcade community and would be keen to see what some of you have to suggest. Your ideas could be in bullet-point format, little description format, or even drawing format...but if you think you can help, send me a pm.

I have been watching some courses on iphone development on itunesU from Stanford. This (http://deimos3.apple.com/WebObjects/Core.woa/Browse/itunes.stanford.edu.2024353965.02024353968.2117035 884?i=1947006292) one may be helpful, I really enjoyed it. It's a bit long but goes over some really good ways to critically test your interface. In it he also mentions a great app called omnigraffle (http://www.omnigroup.com/applications/OmniGraffle/) that evidently has an iphone gui plugin so you can quickly mock up different interfaces at the pixel perfect level.
