I'm pretty sure its determined by the number of downloads. Doesn't have to do with whether people review or rate it. You could have 1000 people download an app and not rate or review it.
But i am wondering why this game has so many people download? There seems many games are much better than this.
If you read the description, it looks like an app for an already popular site that was advertised by people like Ellen, Oprah, the Today Show, etc... People are probably familiar with the site and see there is an app available.
Yeah, when you've got Ellen and Oprah mentioning you on their shows, you're going to see a massive number of downloads... many of them from a demographic that may own iPhones but doesn't normally download games of any kind.
