Reviewers or testers or anyone can download an app even if you haven't released it yet. But I think rating an app you have downloaded with a promocode is impossible. We tried it and it failed. iTunes says something like 'You must purchase an app to review it'

I can second this. If you use a code, you can't rate the app.

Does anyone know about gifting & rating the app?

Last time I check, you can still rate if you receive the app as a gift.
