They should have added a questionnairy file to the mail instead of embedded it.

Hypothetically you should be able to copy and paste the embedded content into any word processing software. Aside from the iOS generic wordpad there is likely some iPad word processor but I am unfamilar with iPads. At worst you might end up having to side track to the library between now and next Wednesday to use their desktop computers.

So where do I get the provisioning profile from?

I am guessing that your iDevice's UDID is not registered in the app/ipa. The provisioning profile is in the ipa already. You can only install it on an iDevice's UDID which you've registered when you applied..

Guys you can open and fill in the questionnaire using the Adobe Reader app directly on your iDevice
I'm also having issues with the photo app, can't make an account to use it
