Depends. You can only get Second Chance once, while you can get multiple explosives in a single run. A nuke near the end of your run does wonders for your score, when the game might be too fast for you to reliably survive if you're using a Second Chance. I wouldn't really put one above the other, but I'd rather get a Second Chance earlier and explosives later.
Are you sure you can't get more then one second chance? It's just a reward from the slot machine, so why wouldn't you be able to get it twice? Or does the game take that off the wheels after you get one? I actually hadn't thought about it before. Maybe it does?
It disappears from the wheel, yes. Look at the wheel after you get it once, it spins kind of fast but you'll notice it's not there. Same thing happens with Head Start and Double Coins (when you get either one, both vanish).
Well, actually it's entirely possible to get Second Chance more than once in a game. The delay between it reappearing is about 5-7 spins. It's not very likely to happen though, as there's a further delay before you can actually get it. I know because I lost at ~1,300m, got three hearts and went on till about 6,500m at which point I had 7 more spin tokens. One of those also gave me 3 extra tokens, and by the time I used them all up the resurrect was back on the wheels. It's really tough to get that lucky though...
Ooh, didn't know this. That could be valuable when shooting for ultra high scores. Too bad past ~7000M I tend to die really quickly anyway

Has anyone ever NOT cashed in their level 15 missions for a badge yet? There is an option to decline but then what happens?
Nothing actually happens. Your rank just stays full stars at 15 with no challenges until you do decide to cash in.

I've played the game for a ridiculous 18 hours, 44 min, 8 seconds - and my high score is still only 4449! I'm level 72 though and working towards my 5th medal. Most amazingly, I'm still really enjoying playing - don't think I've ever got as much fun out of a 69p game! Bigger console games should take note...
Um.... I've played for 1D, 9H, 14M, and 4S, 1093 games, with 594 missions completed. My best is only 5, 341M, and I'm on level 145. I had everything purchased, but when I switched to the 4S I lost all of my coins and purchased items/vehicle upgrades. I'm working my way back to have all of the clothes and vehicle upgrades (already got back all of the magnets, though, because those are the best part )

Nukes send you about 500 extra Meters. Not too too much but it sure helps if you're just shy of a new record.
Just focus on one mission at a time. If they are easier in the beginning you can try for two or even three in one game. Once they are in the three stars just focus on the hardest one. Plus if you ever get really stuck you can always just purchase the mission to comets it but that costs alot.
